We are only able to provide you with an estimate of the cost of your visit due to the number of insurance plans we accept; however, finding out the cost of the visit is ultimately your responsibility. Referral/Pre-Authorizations:
So that we may confirm your information prior to the appointment, please direct your primary care physician to send over a referral.
Prior to seeing a specialist, many health insurance plans need referrals or authorization from your primary care doctor. Preauthorization is additionally required by the majority of insurance plans before any kind of surgical procedure or specialized treatment. It is the patient’s responsibility to get the necessary referral authorization from their primary care doctor.
If a referral authorization is necessary, failing to acquire one will result in your insurance plan refusing to pay for our services. In this situation, all services obtained through our office would be the patient’s financial responsibility.

Payment for Services
All payments, including copays, co-insurance, deductibles, and any non-covered procedures, are due at the time of service. Since we are in-network with so many various insurance plans, each plan has its own policies and benefits. Self-Pay Patients
If you will be a self-pay patient, please get in touch with our office to let us know. The staff at our clinic will help you estimate the visit’s cost including any diagnostic testing you may need. The entire amount of the visit will be due at the time of the visit.
